Authorisation
1. I, the undersigned, authorise my child’s/children’s involvement in Fundamental Coaching Centre’s Tutoring Program.
2. I authorise my child/children to participate in centre-based activities organised for the days my child/children will be attending.
3. I authorise that, in the event of accident or illness, to obtain all necessary medical assistance and treatment for my child/children and agree to meet any expenses attached to such treatment.
Privacy
4. I understand that Fundamental Coaching Centre collects information regarding the family for the purpose of registering the children into the education provided. The information will be used for administration purposes and to contact you in the event of an emergency, or purpose directly related to Fundamental Coaching Centre. It will not be disclosed to any other party except as required by law in compliance with the Privacy Act 2002.
Service Acknowledgements
5. I acknowledge that Fundamental Coaching Centre has a 24-hour exclusion policy for children who are sick, and that my child/children will not attend the program if suffering from an infectious or communicable disease as identified by the Department of Health.
- If my child is found to be ill during the course of the program, he/she will not be allowed to continue to participate in the program and parents will be contacted to pick-up the child immediately. No refunds will be given.
- If parents were to identify that their child/children were sick before attending the
program, Fundamental Coaching Centre will attempt to replace the class to the best
of our ability.
Waiver of Liability
6. Except as otherwise expressly required by law, Fundamental Coaching Centre does not accept any liability for personal injury, property damage or loss sustained by any participant as a result of his or her participation of the Tutoring Program, due to any cause whatsoever unless caused by the proven negligence of Fundamental Coaching Centre, its Directors or employees.
7. If you have identified that your child makes use of public transport or is allowed to leave the centre at the end of the lesson, Fundamental Coaching Centre does not take responsibility for any and all causes that may endanger the safety of the child once they have left the
premises.
Inappropriate Behaviour
8. I fully understand that if my child continuously demonstrates inappropriate behaviour after guidance procedures have been followed, I will be notified and my child may be removed, suspended for a period to be determined or excluded permanently from the Service.
Fees & Bookings
9. I agree to pay for all the days my child is successful enrolled in (and not cancelled within the relevant timeframe), regardless of whether my child actually attends with exception to illness.
10. I understand and agree that if my account is not paid in full by the due date, my child’s/children’s enrolment and booked sessions may be in jeopardy, and may be subject to exclusion from the program.
11. I understand that bookings are on a first-come-first-served basis, and that Fundamental Coaching Centre is under no obligation to accommodate change of bookings.
Agreements
12. I agree that the nominated parents/guardians are the authorised person(s) able to collect the enrolled child. If for any reason either parent is unable to collect the child at the end of the session, it is the responsibility of the parent(s) to nominate and authorise a third person to collect the child/children.
13. I agree that we, the parents, must notify Fundamental Coaching Centre on the day via phone of any inability to collect the child and to provide a nominated person to collect the child accompanied by their personal details: Name and Contact Number.
- Upon arrival of the nominated person, they must present a form of identification, either a Driver’s License or Passport, where it will be documented for future reference.
14. I agree that Fundamental Coaching Centre retains the right to update the price, policies of all their services where they see fit.
15. I, the undersigned, agree to all the conditions of this form, and that all information provided is true and correct to my current knowledge.
